The forms available in the Quick Links section allow you to notify our Credentialing department of the most common therapist changes:  

• Adding a new therapist to your membership using a CAQH ID
• Adding a new therapist to your membership without a CAQH ID
• Other therapist changes including a change in service address, name change or termination
